It includes an assessment of the severity of your concern and combines the optimal clinical solution with your unique preferences to create a customized and curated care path.

Homewood Health has added this brand digital platform on Homeweb, introducing industry-leading features and capabilities to our PAP.

Members, Students, their dependent family members, and staff can continue to access a full range of expert health and wellness tools and resources, with these new customized service recommendations, check-ins, a personalized dashboard, and a new online booking system as part of their seamless and simplified assistance journey. App, desktop, and mobile, Pathfinder meets people where they’re at, when they’re ready.
